Making a Claim

If you would like to make a claim for life insurance, personal-lines insurance, or business-lines insurance, we advise you to contact your policy’s insurer in a timely manner. If you would like support or assistance in the matter, please contact us in one of the following ways:

Required Documents

The insurer may require certain documents on a case by case basis. The following are typically required for any type of insurance claim.

  • Claim form (unless online claims are available)
  • Copy of the policyholder’s latest ID card or passport (if not previously submitted)
  • Copies of supporting documents, if available
  • Copies of receipts or statements associated with the claim
  • Evidence of cited cost or hazard

 

 

Additional Documents

Each line of insurance may require additional documents in the event of a claim. The following documents, but not limited to, may be asked for by the insurer.

 

Life Insurance

  • Physician’s statement
  • ID of beneficiary
  • Proof of relationship
  • ID of policy-holder
  • Medical certificate
  • Police FIR or post-mortem report (in case of unnatural death)

 

Business Lines Insurance

  • Incident report
  • Organization charts and identification
  • Receipts or invoices
  • Employee contracts
  • Financial statement
  • Police or accident reports/statements
  • Medical history

 

Personal Lines Insurance
